The traditional focus of medicine has been on the individual, and rightly so. However, clincians also have a responsibility to the population, to the community that supported their education, and provides the resources they use to prevent, diagnose, treat and care. and this is increasingly recognised as resources are less than the need and demand. The clinician needs to continue with their committment to the individual but all clinicians have a responsibility to use reources well and avoid waste, not to relieve the tax payer but to free those reources for other patients who could benefit.
This is the responsibility of every clinician but some clinicians have additional and explicit responsibilities fro managing resources, becuse they work, usually part time in management, We estimate that there are about 500 such clinicians per million population, about 30,000 in Italia as a whole

tthese aims are achieved by a development programme which is based on the three disciplines of clinical medicine, management and epidemiology, particularly clinical and healthcare epidemiolgy. The special skills that need to be developed include, sistemi development, culture change, knowledge mangement and citizen centred care
